The Lake City Customer Service Center is one of Seattle's seven neighborhood-based city service locations, operating out of the second floor of 12525 28th Avenue NE in the Lake City neighborhood of north Seattle. Managed by the City of Seattle, it functions as a community hub — sometimes called a "little city hall" — where residents can access a range of municipal services including passport application acceptance. It accepts DS-11 forms for first-time passports, minor renewals, and certain qualifying adult renewals.
Unlike a post office, this is a city-run facility staffed by Seattle municipal employees, which means the acceptance process and payment requirements differ slightly. Applicants must bring two separate payments: one check or money order made out to the U.S. Department of State (for the federal application fee), and a second cash or check payment to the City of Seattle (for the acceptance fee). Credit and debit cards are not accepted for either portion. Passport photos are not provided on-site, so bring a compliant 2x2 photo in advance.
The center is open Tuesday through Saturday, 9 a.m. to 5 p.m., with no passport processing after 4 p.m. It is closed on Mondays and federal holidays.
