Applicants should arrive with a completed DS-11 form, original proof of U.S. citizenship (birth certificate or naturalization certificate), a valid government-issued photo ID, two identical passport photos, and payment for the State Department application fee (check or money order payable to the U.S. Department of State) plus a separate execution fee (cash, check, or card accepted at the county). Each appointment typically takes about 30 minutes to complete. The office is closed on official Hubbard County holidays.
